NOTICE OF PRIVACY PRACTICES: Questcare is in compliance with the Health Insurance Portability and Accountability Act of 1996 and the regulations promulgated thereunder (collectively, “HIPAA”). Questcare’s practitioners are members of an Organized Health Care Arrangement (“OHCA”) with each of the hospitals where they perform services for Questcare. Consequently, Questcare’s Notice of Privacy Practices, required under HIPAA, is issued jointly with each of those respective hospitals. Persons wishing to review Questcare’s Notice of Privacy Practices should obtain the joint notice from the appropriate hospital. The notices are available on the hospitals’ web sites, which you may reach via the links located on each hospital page of this web site.

THIS NOTICE IS BEING POSTED IN CONNECTION WITH THE FILING OF AN APPLICATION FOR PERMANENT ALIEN LABOR CERTIFICATION. ALL APPLICANTS SHOULD REPORT DIRECTLY TO SHARON HIRST. ANY PERSON MAY PROVIDE DOCUMENTARY EVIDENCE BEARING ON THE APPLICATION TO THE REGIONAL CERTIFYING OFFICER OF THE DEPARTMENT OF LABOR.
